DR DANNY SHUB'S PRACTICE POLICIES

Please feel free to discuss any questions with me or my staff. The policies summarised here have been put in writing to avoid misunderstandings so that we can focus on treatment.

COMMUNICATION: All routine matters of communication should be directed to my office telephone, which is attended on Mondays, Tuesdays and Thursdays from 8:30am until 3:30pm. Messages left outside of these working hours will be responded to on the next working day.
You can also email my office at danny@dannyshub.com.au. Emails should only be used for brief, informational matters. I am not always able to read or respond to lengthy emails about treatment matters – these need to be discussed in your session.

BILLING POLICY: Payment is due at the end of each consultation and can be made by cheque, cash or credit card/EFTPOS.
Accounts not paid at the conclusion of your appointment will attract a $30 account-keeping fee and you will not be able to make any further appointments until the account has been paid.
There are no exceptions to this policy. Apart from medico-legal cases, patients whose accounts are paid by another party need to organise to bring payment with them on the day. Adolescent patients who attend without a parent need to be provided with a cheque for payment, or the account can be paid in advance over the phone by credit card.

COMPENSATION CLAIMS: If you are involved in a compensation claim including Workers’ Compensation, motor vehicle accident or criminal injuries, your account can be sent to the relevant insurer, however you need to provide a letter from the insurer confirming that you are eligible for psychiatric treatment under your claim. Any accounts not paid by the insurer remain your responsibility. Please note also, that there can sometimes be a gap payment between fees charged and the amount reimbursed by the insurer.

CANCELLATION POLICY: If you need to cancel or reschedule your appointment please give us as much notice as possible so that someone else can be seen at that time. A minimum of 48 hours notice is required to allow us to fill the slot. Unless there are extenuating circumstances, appointments cancelled with less than 48 hours notice will be charged at the usual rate for a session.

MISSED APPOINTMENTS: Appointments missed without notice will also be charged at the usual rate for a session and this account must be settled before any further appointments can be made. Repeated missed appointments will result in the discontinuation of treatment.

REFERRALS: Please make sure that your GP referral is current. Referrals are generally valid for 12 months only and it is your responsibility to keep track of your referral and renew it when necessary. If your referral has expired you will not be able to claim a Medicare rebate for your consultation.

PRESCRIPTIONS: Scripts will only be issued during scheduled appointments. I am not able to post out scripts or fax them to pharmacies.

VACATION/HOLIDAY POLICY: I do not have any set vacations. When I do schedule time away I will do my best to give ample notice. During time away, I will arrange for my practice to be covered by another psychiatrist for emergencies. The covering doctor’s contact details will be provided on my office voicemail. Non-urgent matters should be held until my return.

AFTER HOURS AND EMERGENCY CARE: If you are experiencing an emergency, dial 000 or go to the nearest emergency room.

CONSENT TO COLLECT & DISCLOSE INFORMATION

This Notice describes how health information about you may be collected and disclosed.

The confidentiality of your personal health information is very important to this medical practice.
Your medical record, generally containing information on your symptoms, test results, diagnoses, and treatment, serves as a basis for planning your future care and treatment. We use this record to provide you with quality care and to comply with certain legal requirements.
During the course of your treatment the following information may be collected about you:
  • Full medical and psychiatric history
  • Family medical and psychiatric history
  • Ethnicity
  • Medicare/private health fund details
  • Billing and accounting information
  • Contact details

This information will normally be collected directly from you. There may be occasions when it will be necessary to collect information from other sources with your prior consent. These sources may include but not be limited to:
  • Parents about children
  • Children about their family
  • Schools and teachers
  • Other healthcare providers.

In emergency situations we may have to collect information from relatives or other sources without your prior consent.

USES AND DISCLOSURE OF HEALTH INFORMATION

We will keep your health information confidential, disclosing it only for the following purposes:

Treatment:
We may disclose and/or share your healthcare information with other health care professionals who provide treatment and/or service to you including your referring doctor and to provide referrals to other health care providers or hospitals.
Payment: We may use and disclose your health information to seek payment for services we provide to you.
Emergencies: We may use or disclose your health information to notify, or assist in the notification of a family member or anyone responsible for your care, in case of any emergency involving your care, your location, your general condition or necessary to prevent or lessen a serious and imminent threat to the health or safety of any person or the public.
Required by Law: We may use or disclose your health information when we are required to do so by law (Court or administrative orders, subpoena, discovery request or other lawful process our to meet or obligations of notification to medical defence organisations or insurers).
Abuse or Neglect: We may disclose your health information to appropriate authorities if we reasonably believe that you are a possible victim of abuse, neglect, or domestic violence or the possible victim of other crimes. This information will be disclosed only to the extent necessary to prevent a serious threat to your health or safety or that of others.
Practice management: We may use your health information for practice management matters such as account keeping and billing, quality assurance, practice accreditation and complaint handling.

ACCESS

You are entitled to have access to your own health records at a time convenient to all parties. Depending on the nature of the access requested a charge might be payable where the practice incurs costs in providing access.
There are some circumstances in which access may be denied, but in such an event you will be advised of the reason.
If you find any information we hold on you is inaccurate or incomplete please advise us so we can record your corrections. We are not able to erase the original record.
